Our Nurse-led team work in collaboration with case managers, social workers, and wider multidisciplinary teams to improve the quality of life for individuals with complex health needs. We offer a 24/7 service with short and long visits to suit need, we also provide live-in care and end of life care.
We provide care and support to adults and children with varying needs including those with chronic health conditions or disabilities, encompassing a wide range of physical, neurological, and cognitive challenges that individuals may face throughout their lives. This can include genetic disorders, neurological conditions like epilepsy or multiple sclerosis, as well as learning disabilities such as dyslexia or ADHD.

We have supported our client N since 2007. N has Cerebral Palsy and needed a care team competent in managing PEG feeding, oxygen, oral suction, nebuliser therapy and chest physio 24/7. Our care team has supported N through some very serious chest infections, which have led to lengthy stays in hospital and tracheostomy placement and removal. The care team also supported N to attend a day centre, where N met her best friend A.
When Covid came, the day centre closed so N was unable to attend. Our care team did a fantastic job supporting N throughout this difficult time, with N's confidence and communication skills growing. She was kept safe and well throughout this period and even though the day centre is now open, N does not attend. Instead, she chooses to spend her days going out on day trips, shopping, to the theatre and playing on her Wii console, activities which she is supported with by her care team.
